Up Date 07/27/2012 Dear Friends, Please Download and print out the Registration, Liability and Model Release form below. It's pretty self explanatory. Notes: Only one representative from your band needs to fill out the form. In the "Available Time" slot please put in the earliest time you can play through the latest you can stay, I'll need this information so I can come up with a schedule. If you have more members in the band than what the form can handle please download a second form and just fill in the members names. I'll need this information for wrist bands etc.. The total payment of $ 100.00 stated on the form will be paid to you after your set. The "liability release" is required for our insurance. We will most likely be recording this event and we will be inviting the press . So that's is why the model release is in place. If any part is to be used for commercial resale you will be notified prior to any post production for contract and royalty negotiations. No royalties will be paid for any public media such as news papers and music publications, news broadcast stations and or social media uploads to youtube or other social media sites by us or others.
